There is an area at the end of the Donor History Questionnaire for additional questions, where the donor center may decide to ask questions pertinent to the community in which they are located, set additional requirements for donors that exceed the requirements or recommendations set forth by the AABB or FDA, or add questions as new regulatory requirements go into effect.
For example, the American Red Cross defers donors for additional reasons not on the DHQ:
- Donors who received human cadaveric pituitary-derived human growth hormone are not eligible to donate.
- Donors who have been diagnosed with CJD, vCJD, or any other prion disease, or have a blood relative diagnosed with a genetic CJD, are not eligible to donate.
- Donors who had oral surgery are deferred for 3 days following their procedure.
- Donors with uncontrolled diabetes are not eligible to donate.
- Donors who have ever had Ebola, Hepatitis B, or Hepatitis C are not eligible to donate.
- Donors who have been exposed to measles and are not vaccinated for measles are deferred for 4 weeks from the date of exposure.
- Donors who have sickle cell disease or tuberculosis are not eligible to donate.
- Donors diagnosed with Zika virus are deferred for 120 days following symptom resolution.
