An employee questionnaire about workplace violence prevention should be included as part of a comprehensive prevention program. Questions should cover topics such as, but not limited to:
- Management commitment and employee involvement
- Potential risk factors
- Hazard prevention and control
- Training
- Incidents and reporting
All questionnaire responses should be submitted anonymously to protect the employee. There should also be a section for "Additional comments" to encourage employees to add comments/observations about areas not covered in the questionnaire.
