NCEs of Internal Origin

In addition to data sources originating from outside the laboratory, non-conformities may be identified from within the laboratory or organization.

Some examples of internal sources of NCEs include:
  • Internal audit or interim mock inspection findings
  • Discovery of a defective medical (laboratory) device
  • Quality indicator data
  • Quality control data
  • Re-work data
  • Turnaround time (TAT) reports
  • Training and competency experiences
  • Staff observations
  • Communication breakdowns within the laboratory
  • Mistakes and problems within the laboratory’s work processes
