Donath Landsteiner Test Procedure

The procedure for the Donath Landsteiner Test is outlined below:
  1. Incubate 1 tube of serum combined with P antigen positive RBCs at 37oC for 60 min.
  2. Incubate 1 tube of serum combined with P antigen positive RBCs at 4oC for 30 min. then to 37oC for 30 min.
  3. Centrifuge both samples and observe for hemolysis.
In PCH, the first sample's serum will be negative (clear) while the second sample will be positive (hemolyzed) indicating a biphasic hemolysin.
