The procedure for the Donath Landsteiner Test is outlined below:
- Incubate 1 tube of serum combined with P antigen positive RBCs at 37oC for 60 min.
- Incubate 1 tube of serum combined with P antigen positive RBCs at 4oC for 30 min. then to 37oC for 30 min.
- Centrifuge both samples and observe for hemolysis.
In PCH, the first sample's serum will be negative (clear) while the second sample will be positive (hemolyzed) indicating a biphasic hemolysin.
