Root Cause Analysis

Root Cause Analysis is a process to determine the basic and contributing causes as they relate to an event or combination of events.

The goal of a Root Cause Analysis is to find out:

  • What happened?
  • Why did it happen?
  • What can be done to prevent it from happening again?

The Root Cause Analysis must move beyond "whose fault is this" and focus on the causes in the current systems.
