Severity Assessment

When considering severity, the risk to the following must be considered:

  • Patient Safety
  • Donor Safety
  • Test Result Quality
  • Blood Product Quality
  • Employees
  • Organization

The following is an example of severity rating categories that can be assigned to an event.

  • Catastrophic - death, major permanent loss of function, sentinel event
  • Major - Permanent decrease of bodily function, surgical intervention required, disfigurement, increased length of stay (LOS) or level of care for 3 or more patients
  • Moderate - Increased LOS or level of care for 1-2 patients
  • Minor - No injury or increased LOS
