When considering severity, the risk to the following must be considered:
- Patient Safety
- Donor Safety
- Test Result Quality
- Blood Product Quality
- Employees
- Organization
The following is an example of severity rating categories that can be assigned to an event.
- Catastrophic - death, major permanent loss of function, sentinel event
- Major - Permanent decrease of bodily function, surgical intervention required, disfigurement, increased length of stay (LOS) or level of care for 3 or more patients
- Moderate - Increased LOS or level of care for 1-2 patients
- Minor - No injury or increased LOS
