Documented reactions should NEVER be changed or omitted. The initial reactions obtained must be retained for documentation once ABO discrepancy resolution is complete. Both initial reactions and resolution reactions must be properly documented and stored for later review.
Additionally, it is critical to ensure documentation of all techniques used for the resolution of the discrepancy. This serves to prove proper and adequate investigation and accuracy in the reported patient blood type.
