Dictation Template: Oriented Skin Ellipse

The specimen is received in formalin, labeled with the patient's name, and designated on the requisition as [right foot nevus.] The specimen consists of an oriented skin ellipse measuring [ ] cm from superior to inferior, [ ] cm from lateral to medial excised to a depth of [ ] cm. There is a short suture designating superior / 12:00 margin and a long suture designating lateral / 9:00 margin. There is a [ ] cm blue-brown, endo-exophytic nodular central lesion, which is located [ ] cm from 12:00 margin, [ ] cm from 6:00 margin, and [ ] cm from 3:00 and 9:00 margin. The margins are inked as follows: 12:00-3:00 - green, 3:00-6:00 - orange, 6:00-9:00 - blue, 9:00-12:00 - yellow. The specimen is entirely submitted as follows: cassette A - bisected 12:00 margin, cassette B - bisected 6:00 margin, cassettes C and D - sequential sections from 12:00 toward 6:00.
