Category B Labeling and Marking

UN3373 Category B, Biological Substance shipments need to have the following labels and markings:
  • Any necessary handling labels (e.g., frozen, fragile, etc.)
  • Name, phone number and address for shipper and recipient (“To” and “From”)
  • Name and phone number of person responsible for package. The responsible person must be able to provide emergency response information about the package if needed.
  • Label each specimen container with the patient’s unique identifying number(s) (e.g., medical record number, laboratory requisition number), specimen type (e.g., serum) and the date the sample was collected.
  • List the name and phone number of the person responsible on the outer box.
Apply the required Category B hazard label (UN3373) to the outer box along with the proper shipping name of “Category B, Biological Substance”.
