Most clinical incidents are due to a failure of systems. Identifying and tracking errors is a critical component in continuously improving systems that allow laboratories to provide a patient experience that is safe, timely, effective and centered on the patient’s needs.
Analysis of reported events, including the circumstances and processes involved, provides opportunities to implement preventative actions and mechanisms that include feedback and learning throughout the organization, and to provide the necessary education to prevent recurrence, and reduce the risk of patient harm.
