Performing error follow-up in a Just Culture should always include:
- A critical analysis that takes into consideration the complexity of situations
- Determination of factors in play that "allowed" or even "encouraged" the error
- Constructive feedback
- Fair-minded treatment
When errors involve system issues:
- "Retraining" will accomplish little or nothing
- Not many mistakes really stem from a lack of training or education
