History of Present Illness (HPI) for Danielle

Danielle is a 13-year-old patient brought into the clinic by their parents who are concerned about weight loss combined with the fact that "Danielle is a picky eater". They state that Danielle has lost significant weight since middle school started and doesn't like many foods anymore. They say that Danielle has not started having menstrual periods and they find excess amounts of hair in the shower. Danielle would not provide any answers to the provider's questions with their parents in the room. The parents were asked to leave so that the provider can speak to Danielle alone.
Danielle then gave more information, saying that they had been very concerned about weight since the beginning of middle school. The patient was concerned about their physical appearance compared to other students, so they tried to eat as little as possible. They explain that they do not make themself vomit, they only limit what is eaten even though they know they are skinny already. Danielle states they have felt very “weird” in their brain and have fallen behind in school work because they cannot stay mentally focused anymore, but this is all secondary to the desire to lose weight and stay “skinny.”
