Completing Step 1 of the Federal Drug Testing CCF

A. Verify that the employer’s information is present and correct.
B. Make sure that the MRO’s name, address, phone and fax numbers are listed. The MRO’s address must be a physical address, not a post-office box number.
C. The donor’s name is never to be used as an identifier on the Federally Regulated CCF. Only the donor’s social security number or some other identifying number may be used.
D. The collector must indicate the testing authority or administrative agency.

  • FMCSA – Federal Motor Carrier Safety Administration
  • FAA – Federal Aviation Administration
  • FRA – Federal Railroad Administration
  • FTA – Federal Transit Administration
  • PHMSA – Pipeline and Hazardous Materials Safety Administration
  • USCG – United States Coast Guard
E. Check the reason for testing.
F. Indicate the drug tests to be performed.
G. Note the physical address where the collection took place plus the collector’s phone and fax numbers.
