Current Address

As a laboratory licensee, you are required to maintain a current mailing address with the Board of Clinical Laboratory Personnel and your primary practice location. If you move, or change your practice location (even if it is outside of Florida), you must notify the Board within 60 days of this change. You may do this either by mail or electronic methods (See the Board’s website, http://floridasclinicallabs.gov/ for procedure and change of address form). The reason for this is so that any notices required by law may be served to you by the Board or its agent. You must also designate which of the two addresses should be used to serve you with any notices or other documents. (Refer to Fla. Adm. Code Ann.r. 64B3-1.006)
